Understanding Infant Torticollis

Torticollis is a common condition in infants that causes a baby’s head to tilt to one side while the chin points in the opposite direction. This condition is often related to tightness in the neck muscles, particularly the sternocleidomastoid (SCM) muscle.

Parents may first notice that their baby prefers looking in one direction, has difficulty turning their head evenly, or develops a flat spot on one side of the head. While this can feel concerning, the good news is that infant torticollis typically responds very well to physical therapy—especially when treatment begins early.

At Physical Therapy Services, our outpatient physical therapy team works closely with families to help infants improve mobility, strength, posture, and developmental movement patterns in a gentle and supportive environment.

What Causes Torticollis in Infants?

Several factors may contribute to torticollis in babies, including:

  • Positioning in the womb

  • Limited space during pregnancy

  • Birth-related muscle strain

  • Tightness in the neck muscles

  • Consistent positioning on one side after birth

Torticollis is also commonly associated with positional plagiocephaly, or flattening of one side of the head due to repeated pressure.

Signs Your Baby May Have Torticollis

Common symptoms of infant torticollis include:

  • Head tilted to one side

  • Difficulty turning the head equally in both directions

  • Preference for looking one way

  • Flattening on one side of the head

  • Trouble with tummy time

  • Stiffness in the neck

  • Delayed motor milestones in some cases

  • Feeding difficulties on one side

If you notice any of these signs, a pediatric physical therapy evaluation can help determine the best treatment approach.

How Physical Therapy Helps Treat Infant Torticollis

Physical therapy is considered one of the most effective treatments for infant torticollis. At Physical Therapy Services, treatment plans are customized to each child’s specific needs and developmental stage.

Gentle Stretching Exercises

Therapists use safe, gentle stretching techniques to improve neck flexibility and restore full range of motion.

Strengthening Activities

Therapy sessions include exercises and positioning activities that encourage balanced muscle development and symmetrical movement.

Tummy Time Support

Tummy time is essential for strengthening the neck, shoulders, and core muscles. Our therapists guide parents on effective tummy time techniques that support development.

Positioning Education for Parents

Parent involvement is an important part of successful treatment. Families receive education on proper positioning during feeding, sleep, playtime, and carrying.

Developmental Milestone Monitoring

Physical therapists also monitor gross motor development, helping babies progress with rolling, sitting, crawling, and other important milestones.

Why Early Intervention Matters

Early treatment often leads to faster progress and improved outcomes. Babies who begin physical therapy within the first few months of life frequently respond very well to treatment and may require fewer therapy visits.

Without treatment, torticollis can sometimes contribute to:

  • Persistent head tilt

  • Flat spots on the head

  • Poor posture

  • Delayed motor skills

  • Movement asymmetries

Early intervention helps support healthy growth, comfort, and movement during critical developmental stages.

What to Expect During a Physical Therapy Evaluation

During the initial visit at Physical Therapy Services, a pediatric physical therapist will evaluate:

  • Neck range of motion

  • Muscle tightness

  • Head shape and positioning

  • Strength and movement patterns

  • Gross motor development

  • Feeding and positioning habits

Following the evaluation, your therapist will develop a personalized treatment plan and home exercise program tailored to your child’s needs.
